Athletic Update as of 1/30/21
Thank you for taking the time to read through this update and for getting your athletic clearances completed. Our league has come up with a different schedule than what the state had originally designed for our sports seasons. If you have specific questions about conditioning or practice for your sport, please contact the coach directly. Coaches list Below is information on the athletic clearance process for this year including some big changes to concussion baseline testing and getting cleared.
2020-21 Athletic Start Dates (tentative due to COVID-19):
Season 1: February 1st
Season 2: March 1st
Season 3: March 29th Outdoor Sports, April 5th Indoor Sports
In order to be cleared for each sport, you MUST complete the following 3 steps (also can be found under athletic clearance in the menu above). All athletes need to be cleared by the start date of their sport or they cannot participate. See SCVAL Schedule here.
1. Complete physical exam part 1 and 2:
Locate forms here:
Part 1: FUHSD Physical Exam Pt. 1
Part 2: FUHSD Physical Exam Pt. 2
***All physical forms must be completed by a parent/guardian and doctor prior to the start date of your sport. The state of CA only accepts physicals that are signed, stamped and dated from a MD, DO, PA or NP. Physicals signed by a RN, LVN, PT or DC are NOT allowed.***
Physical forms will need to be scanned and uploaded into your athleticclearance.com account (as one document).
Your physical form must be SIGNED, STAMPED, and DATED.
2. Digital registration must be completed via : www.athleticclearance.com
Registration for all seasons is currently open. Parents and their athletes will need to visit the site, create an account, and register for each sport they plan to play. Returning athletes will need to log in and update their grade and make any other necessary changes.
Please do not register for “All Sports”. Register for only the sports you intend to play. You can always log back in and add sports at a later time if you are undecided.
3. Preseason concussion baseline testing.
UPDATE FOR 2020-21 : FUHSD has temporarily allowed for concussion baseline testing to be completed at home, under the supervision of a parent/guardian. *instructions on how to complete the testing are on page 2, make sure you keep a copy prior to submitting your document.
Visit the link above in order to access information about the test and testing environment. All tests completed without first signing the digital consent at the link above will be marked “invalid” and deleted from the system.
Please take this test seriously as it will be used as your baseline to evaluate possible concussion symptoms in the event of an injury. If this test is not completed prior to the start of your season, you will be held out of all live contact until it is done and logged.
4. Email the Athletic Trainer
Once you have completed the previous 3 steps you need to email our athletic trainer, Katie Newbolt, firstname.lastname@example.org She will check your concussion test for validity and then let you know if you have been cleared for your sport.
